Online appointment requests
Form information updated on 9 October 2026.
The patient's name, contact phone number, year of birth, preferred day and time, and appointment urgency are required. Comments are optional. The form does not ask for your email address. These details help the administrator contact you and arrange the appointment. The urgency choice is a scheduling preference.
Appointment requests are received by the centre's administrative team through the website, email notifications and the clinic's Telegram notification group. Telegram is an external communication service. Please keep comments limited to information needed for the appointment and provide medical records directly to the treating clinician.
If you book for a child or another person, provide their name and a phone number for the person arranging the visit. Please provide these details only when you are authorised to do so.
For questions about your data, corrections or a request to stop appointment-related contact, write to binocular.ua@gmail.com or call +38 097 729 45 05, +38 093 382 81 13 or +38 099 323 71 13.
